Life As A Houseman In HSIJB - Part 3 O&G

Hi everyone, I am going to move to a new website.

Some post will follow and this blog will be deleted.

Do join me for more info about doctors and life.

Thank you

Does anyone interested to join HSIJB for housemanship ? If you do, you can approach me by commenting below. Recently i am thinking regarding life as a doctor...
Are you really passionate to become a doctor? or just because of family pressure? for wealth? excellent in study?

Not surprise, I found out lots of my friends had tired of doctor's life. Becoming a doctor was not easy/relaxing, you had to sacrifice a lot...time, energy, sleep, and relationship till the end of your life.
Hence, lots of my friends started to think to become MO in Klinik kesihatan (KK) / dermatology/ non clinical field, which mean you will have more personal time.

Just let you imagine...you have to work >12 hours per day (without proper lunch time, sleep time (as you have to do oncall/ night shift), and you only get 1off day per week , sometimes 1off day per 2weeks. Then you started to envy your friend had more personal time (they might work from 8am till 6pm, with off day in weekend). Besides, you cant enjoy public holiday as others do , accident happened anytime. You might not able to get your leave application to attend your friend wedding/ travelling / new year. This just a small part of hardship...you had to face life and death, your ignorance might cause harm etc...

Anyhow, let shift attention to O&G department (the most exciting and palpitation department)!!

During my university period, my ambition was become an obsterictist because you can see the born of small baby, less chronic patient, assist in infertile couple etc. However, my dream switch to surgery as i think that O&G was too stressful and highly medicolegal.

In HSIJB, the O&G is the best department. As usual, you need to learn how to conduct normal delivery, performing episiotomy and suturing. If you are active and interested in O&G, you can perform vacuum delivery , twin delivery, breech delivery under supervision. It was not that tough once you knew the principle and you had mastered the vaginal examination.

The unique things here was you can do ultrasound scan! Basically you will be taught in basic antenatal scan. Houseman will be in charge in PAC and all the new case will be discussed with our consultant! Yes, you need to discuss the case with consultant! Consultant ! haha...can you imagine that? You can diagnose type of breech , suspected macrosomia, twin pregnancy, incomplete miscarriage, retained POC, placenta abruptio, hypertensive crisis etc etc here. But of course, the high-five case will be still attended by specialist / MO , but you will be the 1st line.

I can still remember the day a patient come in with ?show but it ended up to abruptio placenta with woody hard uterus (at last the mother and baby was fine). Moreover, you will find joy when your EFW scan findings was almost exactly with the baby's weight! Working in PAC was indeed streesful, but you need to become a safe doctor! You need to have high suspicious. Mother come in with os fully, but my abdominal palpation showed uterus was too larger than date, registar was informed during labour stat, crucial steps done, the baby come out at weight >4kg, shoulder dystocia was avoided.

Lots of exciting things can happened in O&G here. My famous quote "Teran panjang panjang" (push long and push hard) to encourage mother during 2nd stage of labour. You can have unforgetful experience here. Forgot to mention, you have to tag 2week for 1st poster while 1week for 2nd poster above in labour room. The MO and specialist here were nice! You can learn tons of things from them if you are proactive. Your heart will be melted when you see the happiness came from the baby and parents =D

Anyone had the same feeling as me? Feel free to make friend with me!

Coming up next- The posting that fill up with tough men (mostly)! Orthopedic

    4. Thanks alot Vincent. And I would like to know about assessment? How about extension rate in HSI?

    5. Hmm...currently i not sure. But i believed most hospitals (different hospital and department having different extension rate) No need worry about this. As long as you are functional, hardworking and good attitude, you wont be extended easily.
In addition, if you had those good criteria, you can survive easily at any hospital.
For assessment part, u need to read my other posts.

    1. Initial yes. and take longer time to adapt compare to local grad. But after adaptation, not much problems and performance level can be equal. Attitude played an important role.

  11. Hi, I have a question about MO posting. Based your experience, any of your friends from KL did not get to return back to KL after serving out of state as a HO? If yes, how is the process of MO placement?
